Original Document

To have an original document apostilled, you will need to make sure:

  • You have an original document or duplicate issued by the State
  • Appropriate signature is provided
  • Original document was recently issued

Public and private documents can be apostilled with the following:

  • Signature of Patricia Adams or the current Arizona State Registrar
  • Must contain proper notarial certificate including the venue and expiration date must be present in the notary seal
  • Notary’s signature must match respective notary’s seal

  • Documents with the signature of Julie Frasco are not acceptable for apostille.

Scanned Photocopy of Original Document

You will need to make sure of the following:

  • Appropriate signature is NOT provided
  • Original document is damaged
  • Original document was issued over 10 years ago
  • Destination country ACCEPTS apostilles of the scanned photocopy of the original document; most countries do accept an apostille of the scanned photocopy
  • You need the apostille within 24 hours

 Please note:

Because the notary and the apostille are required to be issued from the same state, the scanned photocopy of the original document that you provide will be notarized and apostilled in New York City.

Note that an apostille and/or exemplified duplicate of your original document of vital records issued by the Arizona Bureau of Vital Records can only be requested by:

  • The parent of the person named on the document

  • A person designated in a power of attorney
  • The adult child of the person named on the document who is at least 18 years of age
